PACT Nurse Case Manager

$3,000 SIGN-ON BONUSEliot is seeking a Licensed RN or LPN in Massachusetts to work in the community with adults with severe and persistent mental illnesses.
The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment.
This person would be comfortable with outreach, case management and providing support in individuals homes.
Eliot's Adult Mental Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that promote consumer rights, responsibilities, rehabilitation, and recovery.
We provide rehabilitative services that enable people to increase functioning in the living, working, and social environments of their choice.
Responsibilities Act as a primary case manager to 3-6 consumers Provide nursing interventions to individuals Assist in the management of medications and the overall functioning of the nursing department Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload Actively take part in supervision, staff meetings, in-services and trainings Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
Schedule:
Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall scheduleSalary:
RN -$77,680.
00 annually LPN -$58,073.
