Bachelor level Vocation Case Manager PACT

$3000 SIGN-ON BONUSEliot's PACT program is expanding and in need of a candidate that has case management experience, can develop treatment plans with the individual's participation.
They will possess excellent communication skills, and work well in a team setting.
Prior experience providing vocational counseling and vocational development to people with dual-diagnosed mentalillnesses is needed.
Bi lingual experience preferred.
Eliot's Adult Mental Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that promote consumer rights, responsibilities, rehabilitation, and recovery.
We provide rehabilitative services that enable people to increase functioning in the living, working, and social environments of their choice.
Responsibilities:
Provide case management services as part of a multidisciplinary team Develop treatment plans with consumers related to vocational goals Participate in Daily Organizational Meeting (DOM) in accordance with Eliot policies and PACT standards.
Document daily progress notes for all contacts with each consumer.
Perform shift management as assigned by the Team Leader in accordance with Eliot policies and PACT standards.
Act as a primary case manager to 7-15 consumers in accordance with Eliot's policies and PACT standards.
Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner.
Schedule:
Monday - Friday 8am to 4pm with rotating evening shifts Eliot mandatesCOVID-19vaccinationsforemployees, please discuss the requirements with the hiring manager during the interview process.
Bonus breakdown:
Day 1 (New Staff Orientation) $500 30 days of employment- $1000 6 months ofemployment - $1500 Recommended Skills Assessments Case Management Communication Multidisciplinary Approach Team Working Powdered Activated Carbon Treatments Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
