Substance Abuse Case Manager PACT

Eliot is currently seeking a strong, experienced Substance Abuse Case Manager for our team.
The ideal candidate must have proven ability to develop dual diagnosis groups and provide individualized substance abuse support.
Knowledge of 12 step program.
Must have demonstrated knowledgeof consumer empowerment and consumer recovery.
Eliot's Mental Health division provides anintegrative and comprehensive network of services to adults with psychiatric disabilities that promote consumer rights, responsibilities, rehabilitation, and recovery.
We provide rehabilitative services that enable people to increase functioning in the living, working, and social environments of their choice.
Responsibilities:
Deliver and monitor consumer medications in accordance with Agency, DMH and DPH regulations, policies and procedures.
Ensure the maintenance and accuracy of all documentation and administration.
Transport consumers to work sites, meetings, daily activities, appointments, as needed.
Follow all motor vehicle rules and regulations when driving consumers.
Maintain a positive professional working relationship with consumers, co-workers and interested parties Ensure the safety of consumers; implementing crisis intervention as necessary.
Participate in Daily Organizational Meeting (DOM) in accordance with Eliot policies and PACT standards.
Document daily progress notes for all contacts with each consumer.
Schedule:
Monday - Friday 8am-4pm with rotating evening and weekend shifts.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
