Summary:
The Case Manager performs as an essential part of a multidisciplinary team of medical professionals, clinicians, and Recovery Specialists to provide support, counseling, and community referrals to individuals with substance use and co-occurring disorder, including facilitating individual and group counseling sessions and providing aftercare case management planning services.
The Case Manager contributes to Bay Cove's mission by directly partnering with individuals and coordinating the services necessary for their recovery journey.
This is a non-exempt position.
Essential Functions of Position:
? Orient each new or returning patient to the unit, inform them of their rights, and familiarize them with the community guidelines, procedures, activities, policies, and philosophy of the program, including program requirements for participation, disciplinary action, termination, and grievance procedures as well as providing informed consent and obtaining sign on consent forms.
? Under the guidance of the Clinical Director, engage patients in information gathering and decision making to inform the development of the psychosocial assessment, the setting of treatment goals, determination of service needs, and aftercare planning.
? Coordinate services with external treatment providers, outpatient counselors/therapists, opioid treatment, and other partners such as primary care, DCF, Courts, and assist patients to coordinate and attend essential appointments as needed during their stay.
? Develop aftercare plans that are in line with patient needs, interests, and eligibility, make referrals, facilitate interviews, secure placement, and coordinate a warm transition to aftercare services.
? Provide individual and group education and recovery support appropriate to patient medical and mental status during detox/withdrawal and within the limitation of assessed competency and qualifications.
? Document all services provided and patient response to those services in the Electronic Health Record.
? Work collaboratively with the multidisciplinary team to identify changes in patient wellbeing, progress made, and development of new needs, and needed changes to the treatment plan and document the results of these discussions in a Multidisciplinary Service Note.
? Provide crisis prevention and intervention, escalating to the appropriate party when assistance is needed (supervisor, BEST Team, etc.
).
? Coordinate each patient's discharge in concert with nursing and aftercare providers and complete all required discharge documentation.
? Conduct and document post-discharge follow-up for clients within 30 days of discharge.
? Other job-related activities as assigned.
All BSAS licensed programs must have at least one staff member designated to serve as coordinator of one of the following three topics.
If assigned, both supervisor and employee must initial to acknowledge the assignment:
? Serve as the program's Access Coordinator by developing and implementing the evaluation, plan, and annual review of the program's performance in ensuring equitable access to services as required by 105 CMR 164.

? A High School diploma or equivalent is required.
? A Bachelor's Degree in a social work or related field or equivalent combination of education, training, and/or professional or personal experience is required.
? Minimum of one (1) year experience in Behavioral Health or Human Services.
? Demonstrated knowledge of health care, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology is preferred.
? Demonstrated ability to speak clearly and persuasively in a variety of situations required.
? Working knowledge of word processing and other computer functions using Windows, Word, and Google Email and Apps required.
? Certification or eligibility for certification in substance abuse counseling or willingness to pursue certification is welcome, encouraged, and will be supported.
? An ability to learn and understand federal regulations pertaining to confidentiality of substance abuse treatment is required.

Physical Requirements ? Ongoing detection and interpretation of information on documents, monitors, and labels and accompanying response, as well as to identification of equipment and supplies.
? Frequent interaction with providers, colleagues, customers, patients/clients, and visitors requiring the employee to communicate, as well as detect and interpret information, needs, and issues quickly and accurately, occasionally during emergency situations.
? Frequent communication of accurate information, instructions, and ideas so others will understand.
? Operation of complex and delicate equipment with precision and accuracy.
This includes frequent phone and computer use for documenting patient care, accessing needed information, etc.
? Bending to retrieve, lift, and carry supplies and equipment up to twenty (20) pounds.
? Frequent movement throughout the unit to access, obtain, or distribute supplies (sometimes pushing or pulling equipment), interact with patients, assist in the transport of patients, etc.
? Undertake lifesaving efforts by conducting CPR via the provision of chest compressions and rescue breathing as needed until relieved by medical professionals.
? Requirement to remain in a stationary position for an extended period of time as needed.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
